What do you do if you get into an accident with the vehicle or find new damage on the vehicle?

In the event that there is an accident or new damage is found when completing the Vehicle inspection with an Organizational Vehicle staff will notify their supervisor or On-call depending on the hours. Follow the steps outlined in OMI's Safe Transportation Procedure and complete the Vehicle Incident/Accident Report to be turned into the Operations Director.

What do you do if a staff doesn't show up for their shift?

If a staff does not show up for their shift the staff on duty MUST stay until that staff arrives or they find someone else to come in. If there is two staff on duty and neither staff is willing to stay the staffs who have worked the least amount of consecutive hours must be the one to stay. If both staff have worked the same amount of hours then the two on duty staff should flip a coin to determine who has to stay. If a staff does not show up for their scheduled shift the staff on duty should call them. If they do not answer staff should leave them a message. If the scheduled staff does not answer and the staff on duty is not willing to stay they are responsible for calling all other staff on the phone list to see if anyone else can come in and work. If the staff that was scheduled for a shift shows up or calls back and the shift has already been covered it is up to the staff covering the shift if they want to leave or not. The originally scheduled staff does not get the option to work or not. All staff whose hours are affected by a no-call no-show or a call no-show MUST fill out the shift variation form.

What is OMI's personal cell phone policy?

OMI recognizes that cell phones are a part of everyday life, and that employees bring personal phones with them while at work. The purpose of this policy is to form a work environment that is safe, productive, and free of distractions.

It is the Organization’s expectation that all personal cell phones are on silent mode during the shift to maintain normal workflow and to not disturb the home life of the individuals we serve. It is expected that employees limit personal use (i.e. check it and put it away again). Personal calls and texting should primarily be done during established breaks. An established break is in accordance with the Break and Meal Period policies, 202 and 203, and has been communicated with other staff so that they know the employee is on break.

When using personal cell phones intermittently, employees should do so at an appropriate time as to not disturb the functioning of the home, and not interfere with job duties. Do not use a phone while providing personal cares. Distance should be made between co-workers and individuals we serve so that others do not hear personal conversations. Supervision of the individuals we serve must be maintained. Attention to surroundings must be maintained.

Cell phones are not to be used to record or photograph any individuals served, or any confidential or private information. OMI is not responsible for damage to personal cell phones. It is expected that staff keep personal cell phones securely stored. This might mean placing in the locked office, put in a pocket that has a zipper, or in clothing with a deep enough pocket so that the phone will not fall out when doing personal cares or dealing with an interfering behavior.

Use of ear buds is discouraged, but it is accepted to use one ear bud while the other ear remains open for hearing and attention to surroundings.

The use of cell phones while driving is prohibited except for voice prompts from a map. In accordance with the MN Hands Free Law, map directions must be set prior to moving the vehicle, and if adjustments need to be made it is expected that the driver pull over and stop in a safe location.

When not on an established break, unacceptable use of personal cell phones includes and is not limited to; watching a movie on the phone, playing games, engaging in social media,

continuously answering calls and texts that are not urgent, frequently going away (i.e. bathroom, outside, another room of the house) and co-workers do not know where the other staff is, using the cell phone at organizational events and out in the community with individuals, and/or being fully engaged on the phone while ignoring individuals served and co-workers.

Excessive use of a personal cell phone during work hours may lead to disciplinary action, up to and including termination of employment.

How do you redirect AJ when he is refusing his medications or completing his cares?

Andy will often be non-compliant with new staff when completing cares and taking medications. Explain to Andy why it is important for him to take the medications/complete the tasks. Give Andy time and space and reapproach him. Staff should continue giving space/reapproaching and encouraging him to complete task/take medications. Let Andy know what he can do once he is done (watch TV, go to bed, play games, etc.). Let Andy know that he has the right to refuse but you are going to need to let his dad and Micaela know.

What is BR's brief behavior overview?

When Brett becomes overwhelmed, upset, anxious, or frustrated, he may display behaviors. If Brett is extremely worked up, he needs space and time, talking to him will only cause him to escalate more. Brett has a history of lying and false reporting. Brett has a formal social skills outcome in place located in his program book.

What do you do when ATJ displays self injurious behaviors?

When noticed staff will check the injury and complete first aid if needed providing Alvin with the least amount of interaction as possible. Staff will then avoid any conversations with Alvin regarding the injury.

If GT is writing, saying, or doing inappropriate things to staff (blowing kisses, invading personal space, wanting to marry them, etc.)  at someone how do you redirect him?

If GT is writing, saying, or doing inappropriate things to staff (blowing kisses, invading personal space, wanting to marry them, etc.) staff will tell him that it is NOT appropriate, and they are just his staff. Staff will leave the area if action continues.

What is the OMI Individual Fall Protocol, where is it located, where do you document, and who do you notify?

Staff will use their First Aid Training to assess the individual after their fall. Encourage the individual to stay still and not get up until you have checked them for injuries. Check the individual for bruising and/or swelling. Have them move their limbs and perform range of motion to determine injuries. Ask them if they are having pain anywhere on their body or observe their body language for signs of pain. Whenever an individual has a fall and they hit their head and go unconscious, unable to move on their own or appear to have a serious medical concern, staff will immediately call 911. If the individual has fallen and hit their head but they are not showing signs of unconsciousness and don't appear to have a serious medical concern at the time, staff will follow the guidelines set up on the "Mind Matters: Symptoms of a Concussion" poster and will monitor for signs of a concussion. Staff will seek out medical treatment for the individual if they are showing signs of a concussion. Staff may need to feel the individual's head to see if there is an injury or contusion as many of our individuals have very high pain tolerances and may not report pain. Staff will bring them to their primary physician and/or clinic if an appointment is available. if an appointment is not available they will take the individual to Express/Urgent care to be seen. If there is bruising swelling, complaints of pain at the time of fall or noticed after the fall, staff will need to seek medical attention for that individual, when needed. Staff will bring them to their primary physician and/or clinic if an appointment is available. if an appointment is not available they will take the individual to Express/Urgent care to be seen. This is located in the SH med room on the med cabinet. Staff need to document on the fall tracking sheet, HPN's, and med calendar. Staff will notify their PC/PM Mon-Fri while on duty. Staff will notify their PD Mon-Fri 8a-5p when PC/PM off duty. Staff will notify the OMI On-call system after 5pm, on weekends, and holidays.

What is AJ's seizure protocol, who do you notify, and where is it located?

Administer Valtoco 10mg/spray (0.1mL) one spray in one nostril if he has a seizure lasting 3 minutes or more or if he has more than one seizure in 24 hours regardless of the length of the seizure. Call 911 if AJ continues to have seizures after receiving the rescue medication. Notify his legal rep (dad) within 24 hours, case worker within 24 hours, neurologist notify via MyChart within 3 business days. The seizure protocol is located in the back of AJ's green program book.

When do we complete BR's body check, what is the procedure, and where do you document this?

BR's feet are checked twice daily. Once in the morning after getting in his wheelchair prior to putting his socks on. BR then gets a full body check after his shower. When completing a body check you look for any bruising, swelling, scratches, etc. If BR's feet or legs are hot to the touch, swollen, toes have open ulcers, or any signs of infection staff need to follow first aid procedures and notify the LSP, PC/PM, or PD depending on day/time immediately so that an medical treatment can be obtained. If BR's legs are a little swollen or warm staff can wrap BR's legs/feet with the ace bandage and then monitor for symptoms. Anything noted on the body check needs to be document in the HPN's daily until it is resolved.

Alvin has a formal social skills outcome to address his behaviors. This is located in the outcome section of his green program book. Alvin typically displays behaviors as a way of gaining staff’s attention and/or if he is jealous of attention given to others. He also typically will have behaviors if he is bored. Throughout the day staff will engage with Alvin to provide him with positive attention. If Alvin appears bored staff should offer him things to do (shred, bike, clean, games, etc.).

Gary has a formal social skills outcome to address his behaviors. This is located in the outcome section of his green program book. Gary typically displays behaviors as a way of getting staff’s attention and/or if he is jealous of attention given to others. When noticed, staff will provide Gary with a verbal prompt to stop and that his behavior is not appropriate. Staff will provide Gary with as minimal attention for his behaviors. Staff will physically intervene if Gary’s health or safety becomes compromised.
