What expectations do facilitators share with the CP before the conference? (Hint: there are 7 expectations)

  1. All stakeholders must attend the entire conference within the allotted 2-hour timeframe (Please ensure that CP and family are available on that day and time). Families are encouraged to inform their members about this expectation.
  2. Families may have a grace period of 30 minutes for conferences before 5:00 pm and 15 minutes for those after 5:00 pm, subject to availability.
  3. Ensure the presence of the ACS worker and any relevant supports or family providers.
  4. Collaterals must be submitted to the facilitator at least 24 hours before the conference.
  5. Case planners should come prepared with action items and be ready to address safety, risk concerns, and other agenda items.
  6. Speak with the facilitator 15 minutes prior to the conference to share relevant updates.
  7. Debrief with the facilitator for 15 minutes immediately after the conference and complete the STRIVE survey.

While prepping for an STC, you notice there is an open ACS investigation. What do you do?

Reach out to CP, see if ACS is in alignment with closing, if so, see if we can get ACS to document that. If not, see if ACS can attend the conference. If neither is possible, reschedule the conference after the investigation has been closed.
