Call the PCM to discuss possible recerting the patient. 

If we are not meeting the expected outcomes we should stop and discuss the need to continue service. PCMs will know if we have enough supporting evidence for Medical Necessity to recert.

Read ALL PCVA, read ALL orders, read the last few visit notes for each patient and know what's been going on. 

Being prepared for your visist is the most important thing you can initially do for the patient. Great continuity of care begins with being prepared.

ALL prescriptions and ALL over the counter medications, pills, liquids and creams. 

Reminding them about any medications they might keep in their bathroom, bedside, kitchen, or by their chair.

What is the exptected sych times per policy for regular visits, and Oasis Visits? 

Per Policy Regular LPN/RN/PT/PTA visits 4 hrs or less. Oasis visits 24 hrs or less. 

Oasis visits that are not synch back within 24 hours are pentalized $340 then 33% each subsequent day. It is the expectation that EVERY OASIS are synch back within 24 hrs. If you are not meeting this time expectation....START
