Answer needs to be exactly what is in the SOP.
1. Last Name, Form ID#, (Renewal Date, Change, End)
Only include a date for the renewal date. If SA request is to change or end, a date is note needed in the subject line.

The safety plan will outline what the client agrees to do should they begin to experience the desire to engage in self-harm.

No. Per Bestview Employee Handbook, CMs with a full caseload are expected to bill 30 hours per week. It is great that the CM is overbilling, but this does not mean that CM can spend the last few days of the month on correspondence. CM need to be working on something.
