BP 96/90. HR 128, spo2 94%. RR 22.  Veins in neck are distended as patient sits on stretcher in fowlers position.

This rare breath sound is heard over the trachea and upper sternum. it is marked by a loud expiratory sound that is high pitched. It is indicative of fluid consolidation, a space that is usually contains air now has gross fluid.

Apply pads, be ready to shock. Fluid bolus. If transport is long call OLMC for amiodarone drip (150mg over 10 minutes).

Was told to stop taking blood thinners for three days.  BP 170/98, HR 100, spo2 86% RA. 12 lead shows no elevation but depresiion in v1, v2, v3, v4.  Lead I has a large S complex, lead III has large q complex and an inverted q wave.
