The Stage 4 is when the ulcer extends through the skin and the subcutaneous tissues, MAY involve bone, muscles, and other structures. Unstageable is when a full thick area that is covered by slough and/or Eschar in the wound bed cannot be staged, the ulcer cannot be evaluated and the wound will not heal until they are removed.
