If a client is awake in their room and begins to self harm. What should you do?
Provide the client with support until they stop self harming (i.e. utilize touch control, empathic responses). Contact on call clinician. Submit an incident report.
Why is it so important to receive a verbal "Yes Ma'am/Sir understood" after reading the Matrix?
This provides the client with the opportunity to acknowledge the expectations outlined for them and express their agreement and intent to actively follow through.
How often should a bed check be performed if a client is awake, in their room??
Every 5 minutes until client falls asleep, then reverts to every 15 minutes.
What is OneQuest personal device policy??
Staff are not permitted to use a personal device (i.e. cellphone, tablet, apple watch) within the units. Unless prior approval from HR.

Why can't we use "bribes" such as outside food (candy/snacks) to keep clients in programming?
Using bribes disempowers you as a staff member and creates inconsistency within the team. While it may seem effective in the moment, it does not address the root behavior. It only delays it. Bribes are not an appropriate intervention and should not be used to prevent or manage behaviors. Instead, we must remain consistent, uphold expectations, and utilize approved behavior support strategies.

Why is it important for us to ensure we are reading directly from the PBIS Matrix and not by memory?
We read directly from the PBIS Matrix so clients clearly understand the expectations being set. This prevents anything from being skipped or forgotten and ensures we are all delivering the same message. Having the paper in front of us keeps us consistent, thorough, and fair while setting clear structure for the clients.
