Examples of what should go in check in notes

Some Examples:

• Patient in pain (or other concern) and would like to get in sooner
  • This note should be placed any time a patient has been provided the Walk-in Scripting
• Mary in the office approved 3 family members on the same day
• Free/$29 NPA offered or any other pricing discussed (for pricing, make sure you note that you advised it was a starting price)

  • Examples:  “$29 NPA offered.  Simple extraction starting at $209 discussed.”

• Patient is pregnant
• Patient has no teeth and is interested in implants
• Grandparent is legal guardian to minor, will bring in documentation
• Patient states they are HIV Positive
• Family member of patient says that patient has drug abuse problem and wants to make office aware (Example:  “Patient’s mom stated patient has drug addiction”
• If patient asks to note medications on file:
  • First explain they will be able to do that on their first visit
  • If they still want it noted, you may add to the check in notes

If a patient mentions that they don't have a photo ID and they want to know how that will affect them:

• If they do not have insurance and plan to be self pay:

  • Let the patient know that it shouldn't cause an issue for the first visit, and then the office will discuss with them if and when an ID would be needed moving forward

  • Place in check in notes that patient does not have photo ID

• If they do have insurance:

  • They will need to bring their physical photo ID with them (not a picture of their ID).  The ID will need to be state or federally issued and valid (not expired)

  • If they do not have a valid photo ID:

    • They will need to bring with them their birth certificate and/or social security card along with some sort of utility bill/other bill with their name and address on it

    • If they cannot provide any of these things, we are unable to bill their insurance

  • If they say that they do not have an insurance card, advise the patient that they can reach out to their insurance company to get the member number/subscriber ID
