What are Functional Behavior Assessment and Behavior Intervention Plan?
A parent presents the District with a physician's diagnosis and demands services and accommodations through an IEP.

This document must be reviewed annually but does not require a formal meeting to do so.
What is a 504?
The requirement for a student to receive services and instruction outlined on IEP during breaks or summer, due to deficits in progress, documented evidence of the need for continuation of service, or inability to regain progress after missed instruction.
What is Extended School Year?
This process is not a requirement for the District to suspect a disability and the evaluation process to begin.
What is the Pre-referral process/RTI?

What are Adapted Physical Education and Orientation/Mobility?
A student on an IEP is being recommended for suspension of 5 days, putting her cumulative days to 13. (5 days ISS and 3 OSS days earlier in the school year).

What is Specifically Designed Instruction?
As District rep, you are in a meeting and the parent is concerned because a draft had not been sent prior to the meeting as well as sections of the IEP look incomplete or do not provide information for data-driven decisions.
Reschedule if time allows

Sign section14 as meeting participants and continue with the discussion. Reconvene the meeting once the draft is complete

This form is required to be completed and shared with parents/guardians on at least the same timeline as same-aged peers.
