What is: V1 fail, incident report, Managment opens a New Vial Same Lot (V2). Test on initial Refract. Pass move on. Fail, IR, Take Refract OOS move to a different Refract, retest V2. Pass move on. Fail, IR, Managment New Vial Different Lot/Ship Date (V3), try refract again. Pass, continue on to all refracts including initial refract. If initial refract passes on V3, place back into service. Fail, keep trying new lots until passing result.
