First time donors who do not meet the allowable exceptions per SOP (e.g., Toyota, hospital drives, high school students, etc.), must provide a government or state issued ID that includes at minimum...
• A photograph
• First name
• Last name
• Date of birth
Fill in the blanks:
Upon beginning the screening procedure, staff must verify the donor's ________ ___ ___ and ensure it matches what appears on the ____ ____ of the Screening Detail Processing Screen in ABO Wheels.
Full name and DOB
Left-hand side/pane
Fill in the blanks:
Upon beginning the phlebotomy process, staff must verify the donor's ________ ___ ___ and ensure it matches what is displayed in the __________ of ABO Wheels.
Full name and DOB
Left-hand side/pane
TRUE or FALSE
Failure to perform proper verification at any point during the collection process is unacceptable and preventable.
TRUE.
Staff should slow down/take their time to ensure proper and thorough verification is performed at ALL times.
Fill in the blanks:
When verifying the donor, staff should ___ ___ _____ __ _____ their first and last name and date of birth and confirm it matches what is displayed in ABO Wheels, OR verify it by comparing the donor's __ to ABO Wheels.
"ask the donor to state"
"ID" - remember, when comparing the ID to ABO Wheels for verification, it must be obvious!
Upon beginning the screening procedure, staff must verify the physical unit number stickers match what?
The unit number displayed on the donor ticket (KBC-196) and the unit number listed under “TODAY’S DONATION” on the left-hand pane of the Screening Detail Processing Screen.
Staff must verify the unit number stickers match on which items?
• KBC-196 (donor ticket)
• ABO Wheels (left-hand pane)
• Blood bag (set on back of bag, sticker on front of bag)
• Diversion pouch
• Each vacutainer tube
In addition to the donor's full name and DOB, what must be checked during pre-phlebotomy verification and review (specifically for WB procedures)?
Hint: 5 things should be verified for WB collections, in addition to the name, DOB, and unit numbers.
• Review Donor Comments for phlebotomy instructions (MNVL, LAOP, etc.)
• Verify the product/bag label applied appropriately
• Verify the blood type was documented on the bag label correctly, if applicable
• Verify the pregnancy status in ABO Wheels, and that the blood bag label was completed appropriately
• Access the supply icon and ensure the lot number and segment number match between the blood bag and ABO Wheels
Fill in the blanks:
If the donor has registered within the past __ ______ (verified by reviewing the Eligibility tab), it is acceptable to ask the donor if their address or any of their contact information ___ _______ _____ ___ ____ ____ ____ ____ __ as verification of general contact information (e.g., address, phone number, email, etc.).
12 months
Has changed since the last time they were in
TRUE or FALSE:
If there is a wait for a bed/phlebotomist or the donor is not ready to be processed, it is acceptable to place the donor's materials on a phlebotomy station/table and have the donor wait in canteen until the phlebotomist is ready to call them back.
FALSE.
Per SOP 50-71-001, do not leave the bag/paperwork on a table/cart if there is not a clearly defined area indicating which bed it belongs to, or if the donor is not immediately seated on the indicated bed. If there is a wait for a bed/phlebotomist or the donor is not ready to be processed, place the donor’s materials in the containment system. If necessary, direct the donor to a waiting area or provide other instructions as needed.
If the donor changes beds for any reason (e.g., phlebotomist prefers other arm, donor is being restuck, etc.), staff must verify:
a. Name and DOB
b. Unit number sticker on front of blood bag and sample tubes only
c. All unit numbers (sample tubes, blood bag, Wheels, KBC-196, etc.)
d. Nothing, as long as proper verification was initially performed
e. a and c
f. a and b
E.
Staff must re-verify the donor's name, DOB, and ALL unit numbers if the donor moves from the bed.
Fill in the blank:
If it is determined that the _____________ is incorrect or does not match the donor attempting to register, contact the BC On Call group or the QA On Call group to obtain approval to update the information.
Date of birth
When searching for a donor in ABO Wheels, if a match is not found for the donor within the Soundex or Alpha results screens, what must staff do prior to selecting NEW DONOR and creating a new donor account?
Question the donor to determine if they have ever gone by a different name (e.g., maiden name, middle name, nickname, etc.)
When reviewing existing Donor Comments (e.g., HTLG, CANC, TRAV, etc.), staff must verify the text with what, when applicable?
Verify with the donor and the corresponding job aid (e.g., EJA, Malaria Eligibility Guide, etc.).
At minimum, how many times should donor/unit number verification be performed during the phlebotomy process?
4 times!
1. Upon receiving the donor/materials.
2. After filling the sample tubes from the diversion pouch
3. When performing unit number scans at the end of phlebotomy
4. Upon taking the unit and sample tubes to the processing area after collection
Scenario:
Upon beginning the screening process, the screener ("Bob") scans the unit number from the unit number label set into ABO Wheels. After verifying the donor's name and DOB and comparing it to what is displayed in the left-hand pane, the screener advances to the consent and proceeds with screening.
What did Bob do wrong in this scenario?
1. Bob scanned the unit number label set into ABO Wheels. Per SOP 50-71-001, he should have scanned the unit number from the donor ticket rather than the label set.
2. Bob did properly verify the donor's name and DOB, but he did not verify the unit numbers by comparing the unit number set to the left-hand pane of ABO Wheels and the donor ticket on the KBC-196.
If a repeat donor presents and has not registered in the past 12 months, which fields must be verified with the donor (along with the donor's name and date of birth, of course)?
• Full address (including street, city, state, zip)
• Employer
• Phone number(s)
• Email(s)
If at any time a person other than the primary screener performs any initial or repeat testing, the person performing the testing must:
1. Be logged into their own ABO Wheels account
2. Perform proper name and DOB verification
Name and DOB verification is required at any point someone other than the original phlebotomist performs a task in the collection process. However, due to the urgent nature of ___________, name and DOB verification may be completed after this is performed, but should be done prior to documentation in ABO Wheels.
Adjustments
Scenario:
The phlebotomist ("Jane") was busy with another donor and asked for assistance with disconnect. "Marcus" kindly offered to disconnect Jane's donor and proceeded to her phlebotomy U. Marcus asked the donor to state their full name and DOB then logged into his own ABO Wheels account and documented the stop time and volume in ABO Wheels after applying a grommet to the line. He pressed SAVE and then proceeded to disconnect the donor per SOP.
What is wrong with this scenario?
Marcus should have already been logged into ABO Wheels with the donor's account pulled up when verifying the donor's name and DOB. By not having the donor's account pulled up during verification, there is no way he could have properly compared the name/DOB to what was displayed in the left-hand pane as required.
