These three health requirements must be updated yearly to be in good standing with VFI. 

What are the Health Assessment, the TB Risk Assessment, and the Mandatory Flue Vaccination. 

Please note that all employees are required to get a flue vaccine or have a declination and medical exemption form on file. For employees who have NOT been vaccinated against the flu, they must where a mask when with any client or participant.

This is the internal system that VFI uses to keep track of and measure attendance. 

What is the point system. 

Additional notes: VFI employees that are on probation (their 1st 90 days) will be held to a 10 point attendance system. After the probationary period, it shifts to a 2- point system.

VFI provides over 10 examples of unacceptable conduct for employees. These are 5 of them. 

What are: Insubordination, Dishonesty/theft, Neglect Violation of HIPAA, Violation of Safety Policies Failure to report personal injury or property damage, Unprofessionalism, Failure to maintain mandatory health requirements, Failure to attend mandatory in-service(s), Falsification of documentation, Failure to accurately report time, or Violation of any policy/procedure.

All VFI employees are required to complete annual and semi annual in-service trainings, depending on their role. This is the website to access that training:

Additional notes: Training is considered a scheduled work shift. PCA’s are required to complete an additional 6 hours of in-service training annually to maintain their educational certification. Nurses are also required to attend the PCA in-service training. Failure to attend any required in-service will result in termination of  employment.

This VFI Department should be notified of any leave of absence. 

What is the Human Resources Department. 

Additional note: Notify HR of any absence for medical  reasons (in excess of three consecutive days), FMLA, or  otherwise as soon as the need for leave arises. Complete necessary paperwork for disability claims  and/or FMLA. Submit required documentation and updates to HR, including updated medical notes and return to  work notes.

In order to drive a VFI vehicle OR transport a client, these criteria must be met:

Depending on your roll one of these departments should be contacted in order to communicate both foreseen and unforeseen attendance occurrences.

Falsification of time falls into these two categories. 

What are signing in or reporting time of arrival or time of departure for self or  another employee, or requesting another employee to register time on a time card other than what was actually worked OR completing a note claiming you worked a shift when you did not. 

Additional note: All instances of Falsification of Time are grounds for corrective action up to and including termination of employment. Occurrences of Falsification of Time will remain on an employee’s personnel file  permanently, and are not subject to the rolling calendar period for corrective action.

All VFI personnel will receive an annual assessment of their performance and effectiveness. When applicable, this will be a required part of that assessment.

VFI is a drug free environment. True or false, VFI can require employee drug testing and require compliance within 24 hours?

What is true. 

Additional note, employees in violation of the VFI Drug Free Workplace policy must submit to a drug test within 24 hours and will be suspended pending those results. A negative test will result in the suspension being lifted and a positive test will lead to a Last Chance Agreement.

Employees must maintain a base schedule. Failure to work for a period of 4 consecutive weeks will result in this. 

What is termination. 

Additional notes: Failure to maintain a base schedule as discussed in the Mandatory Base Schedule Policy is grounds for termination. This does not apply to any period of suspension, or any absence listed under Attendance Exceptions.

Employee personnel file information includes name, address, phone number, emergency contact and may include driving info and certificates and licenses. It is the VFI employees responsibility to make sure this information is up to date. This department should be contacted in order to update this information.

Additional note: Employees must notify Human Resources of any changes/updates to their information or go to their Paycom self portal to update the information yourself.

In addition to wearing clean clothing that is in good condition and appropriate undergarments, this should be worn at all times while on duty with a client.

Failure to notify VFI of an inability to report to work prior to your shift is a considered a NO SHOW. This will happen for your first NO SHOW occurrence and this will happen with the second. 

What are a final warning and then termination. 

Additional notes: No Show occurrences are considered for action purposes for a period of 12 months from the date of the occurrence. Employees are required to report all absences and attendance related matters to VFI.

Threatening to leave a client is a serious offense. These are the steps that will be taken should someone make that threat.

This system is used to log the time that the employee has worked and includes time in, time out for lunch and back in for lunch, and time out at the end of their shift.

These 5 steps should be taken if you are injured on the job. 

What are report the accident, incident, exposure within 24 hours to HR at extension 305. Call Medcor if the injury needs care beyond first aid. Complete necessary paperwork. Receive proper evaluation, treatment, and follow up as necessary. Submit required documentation and updates to Human Resources.

Documentation refers to the service notes staff members complete  to record what was done during their shift. Documentation can and should occur throughout the  shift, however, this is the timeframe in which all notes should be completed after the shift.

Many Venture Forthe employees may spend the majority of our day on their feet. It is imperative that employees choose  adequate footwear for the amount of standing and walking they may do. These are examples of appropriate footwear.

What are running and/or training shoes with ample arch and ankle support., orthopedic/ergonomically fit shoes, closed toe shoes/boots designed with a non-slip sole (for use in adverse weather).

Additional note: Footwear that is not recommended includes flat soled, non athletic shoes, any shoe with a lift and/or heel, boots without a non-slip sole. Crocks may be worn WHILE working, but should NOT be worn walking to and from client homes or in the community, especially in the winter.
