Document in PCC, this would go under an incident report as physical aggression.  When in the incident, I will make an incident note so it will link to their chart.

RA must reposition resident to alleviate pain, get off of area of concern.  Get Med Tech to examine.

It is everyones responsibility. As an RA, you are first in.  If you see an issue, fix it.
