Participants can send documents up to 60 days after the audit close. Documents received within 60 days will be reviewed and do not require a case. For documents received after 60 days, forward a case for documents to be reviewed.

What is the amount of the tobacco user surcharge? How can EE have this removed?
15% surcharge for tobacco users. Employees who complete the Quit for Life tobacco cessation program can have the tobacco surcharge removed. Wayne Farms will notify ADP as they receive confirmation from Quit for Life and ADP will update the enrollment.

If an Hourly EE is transferred, is there a break in coverage? When can they make elections?
Transfers - No break in coverage. The employee will have a 30 day window prior to their benefit effective date to makes elections. The Eligibility date for New Benefits start on the 1st day of transfer.

Approved appeals will be routed to -NASC-HW-Tier 2. ESR III contacts employee and informs of the approval and takes any further action required to resolve the SR. Client is responsible for the communication of any appeal which is denied.

Basic LTD and Supp LTD. Basic is paid by ER, Supp is paid by EE. EOI is not required for basic, is required for a non new hire for Supp LTD. Max benefit for both is $230,000.

Both Groups: If an EE calls wanting to know who is listed as BN and how to change designation, what steps do you take to assist EE?
ESR advises this information cannot be given over the phone and escalates case for Confirmation Statement. ESR informs employee that beneficiary forms can be obtained from their HR rep. ESR advises the team member to fax the completed form to 801-956-4519
