Indicate if high risk status has been resolved and if no further high risk follow-up is needed OR indicate when next follow up should take place

If an applicant is missing required proofs, can they be certified?

NO. If an applicant is missing more than one required proof, the applicant cannot be certified, even for a shortened certification period. LA staff must advise the applicant to return to the clinic and re-apply once they can provide the necessary proofs of WIC eligibility.

Lactose-free milk: .75 gallon, and one gallon containers AND/OR Powdered dry milk in 9.625 oz containers.

In alignment with federal guidance, what should be followed regarding missing appointments and benefit issuing? 

All participants must be issued benefits for their entire certification period regardless of whether or not they miss their appointment(s).
