Yes! Once the IPOP is signed by all parties, includes all current attachments, and the Staff Action Plan is approved in myEvolv, you can send the packet in!

Document the refusals, commend alternatives, and discuss changes in the upcoming life plan meeting.

Yes. The IPOP needs to reflect the change. Even if it is temporary.

The person supported asked for a change, change in health status, a new goal, relocation, changes in providers, significant changes in the safeguards, when the Life Plan is ineffective, adding/removing waiver services, changes that impact billing (effective period start dates, etc.)
