Admit RN arrives to eval/admit visit with minimal information from Hospital Referral - basing information off of pt and CG report. Pt states he has anginal pain while doing yardwork, but is comfortable at rest.

Pt is ambulatory, can transfer without any assistance. Able to bath independently, has an ostomy bag but is continent of urine. He is able to dress himself, feed himself and cook for himself.

Pt has HTN CHF, but is able to complete gardening activities w/o rest periods. Pt also able to walk around grocery store independently to complete household errands. Oxygen is available PRN, but pt denies using it since SOC.

What is 3/6 ADLs?
Pt presents with disabling dyspnea at rest, requiring 3L of O2 continuously otherwise will have hypoxemia at rest. However, does not have any chest pain. Bed to chair existence with fatigue upon exertion, Lab results show FEV1 at 25%. Pt also has had a history of COVID 2 months ago.
