Indications of use: Afib, DVT, Pulmonary Embolism

Monitor Labs
INR should be less than 1
If taking medication levels should be between 2-5
Vitamin K is the antidote
Warfarin
Inhibits clotting factor Xa
Given SubQ or IV
Monitor aPTT or Anti Xa
Antidote: Protime Sulfate
Side Effects: Bleeding
Heparin
Xa Inhibitor
It differs from Heparin because there is no routine clotting factor. Only give SubQ. Give in the abdomen. Do not expel bubble. Do not massage.
Monitor BUN & CREATININE.
Antidote: Protamine Sulfate.
Enoxaparin/Lovonox
Prevents MI, CVA, chronic afib, relieves pain.
SE: GI ulcers, bleeding, nephrotoxic rheyes syndrome. children should not take. stop 5-7 days before surgery.

to prevent MI, CVA, or manage cardiac stents
