To file a formal complaint: • Use the Harassment & Violence Report Form • Complete all the required details. • Provide the completed report to your Supervisor/Manager or Human Resources. • Should the complaint be regarding your immediate supervisor or department manager you may submit your written complaint to Human Resources or the Chief Executive Officer. • If the complaint is regarding the Chief Executive Officer you may submit your written complaint to the President of the Board of Directors.
