1)Date stamp enelope or 1st page of transcripts and initial
2)Write Cabrillo Student ID #
3)Place in the "Evaluator" folder.
To change name on student account student will need to log in to WebAdvisor eforms and submit a “AR- Change of Information.” Student will need to include a copy of their Driver License and signed Social Security card. If information is not provide student “preferred name” will be changed as requested. The name listed on student change of information will be on all class roster.

Click either "U.S. students apply" or International students apply
(U.S. students will go to OpenCCC website to fill out Admissions application, International students download application and fill out)
