TRUE OR FALSE: My active IFSP family under the age of 3 is moving to another home in WA state. They want to transfer services. I have to complete an exit COS before transferring to the new agency.

FALSE the receiving agency in WA state will complete the Exit COS when the child ages out OR if parents change their mind and wish to decline all services.

(Only when a child is moving out of state or wishes to discharge all services do we complete the Exit COS...and it's been at least 6 months!)

When should the FRC complete the Entry COS during the initial IFSP?

Couple of options...

1) provider pauses in between sections while reading the evaluation: after social emotional, communication/cognitive, and then motor/adaptive (Ideal)

2) FRC can complete it after reading through full evaluation
