False. You should document all incidents you encounter as a CA, such as: - intoxicated students vandalizing property. - students needing medical assistance. - observed pattern of concerning behavior. - any situation where you requested assistance from safety and security.

No. Include as much information as you can in your duty log. In case of emergencies or if you are not really certain about something, you can contact the RC on call and Safety and Security(Security is 5554 and 5555, while the RC on call's number will be in the duty phone).
