Case management system and we can place a referral to families by referring them to a program inside CTI

first 45 days and when a child turns 2.9; for babys under 1 every month we complete vision

Together at the beginning of the month during weekly hb group supervision
biweekly for 60 minutes or based on the familys need; if they have a sibling that is enrolled it would remain weekly

HBM will send out a letter to families and inform home based manager wwe will need follow up in 2 weeks or a FCM will be held with family to address concerns

What is the process in creating goals with families and how many should each parent have? How do we support them in that process. Where do we document?
At least 1 goal best 2 if possible and we support them by using different tools such as pre-assessment, scales and ladders and their input. We create achievable goals with step by step responsibilities and document it as an event under self sufficiency goal
