Individual has a documented diagnosis of a SMI or cooccurring SMI/SUD
Individual was homeless minimally 30 days prior to incarceration
Individual continues to be homeless upon his/her released from incarceration
Upon release, individual will live outside.
Individual does not have public or private health benefits.
HOPE/PATH staff must document the necessary information demonstrating the individual meets HOPE/PATH eligibility. Use assessment tools and/or A copy of the documentation of a SMI or COD must be obtained within 3 months using a clinical assessment or treatment discharge summary from a prior SMI/COD service provider or hospital. A copy of documentation will be included in the Hard Copy Folder.

The purpose of outreach is to identify those individuals potentially meeting PATH eligibility who are living outside and engage in a conversation to determine PATH eligibility. Outreach is face-to-face with the individual. Telephone contact is not counted as outreach.
It is required for all contacts with and on the behalf of the enrolled individual to be documented on the HOPE/PATH Service Note. The service note describes all services as specified on the HOPE/PATH Plan as well as any issues or events with the individual. Outreach contacts are not documented in the HOPE/PATH Record.
