The consumer orientation checklist helps the family understand the expectations of the program. Needs to be updated at a minimum of every 365 days.
This section is located in the BPSA and requires perspectives from the individual, family, and clinician viewpoints.

Strengths, Needs, Abilities, and Preference. Identify from all three persepctives.
When medications are changed, this document needs to be signed before we can notify the PCP.

If a parent is having phone issues, calling and texting may not work, so other alternatives must be used prior to closing a case.
This form gives permission to coordinate care with primary care doctor and schools.

This needs to be done annually to allow coordination with the school, if a client is home schooled, it needs to be noted in the BPSA.
If a release for a PCP is not current, medication letters, program changes, hospitalizations, a health care coordination letters can not be completed.

What is 365 days?
The health screen and the BPSA update needs to be completed prior to the IPOS to justify medical necessity and continued services.
Coordination with this entity should support medical necessity.
What are schools?
Coordination of schools demonstrate the need for services. At our level, clients typically have issues in numerous life domains and this supports medical necessity. When visiting a school, be sure to include talking to staff.
Quarterly reviews are completed within what time frame?
What is 80-90 days interval?
Quarterly reviews must be done within 80-90 day time frames and are stated in the IPOS. This is a good time to review progress from clinician and family perspectives. Give a combination of quotes and ensure there is an update for ALL objectives. Complete CAFAS to show progress.
Discussion of terminating services begins at the _____ appointment.
What is initial or first appointment?
Letting families know at the first appointment that we are planning for success, what that looks like, and services are not forever are ways of developing rapport and engaging with families. Termination begins at the first session.
Prior to a medication being prescribed, a parent/guardian must sign this.

When a new medication is prescribed, the consent needs to be signed stating that side-effects and education have been provided regarding the medication.

What are BPSA, RPP, IPOS.
These documents are updated to explain what led to the hospitalization, what needs to be done differently, and demonstrate increased contact needed.
These are people that support the client and are not paid to do so. Should be documented in the IPOS.
What are natural supports?
Natural supports include parents, friends, relatives, volunteers, church members that assist in meeting the client/family needs and should be included in the IPOS.
Part of engaging a family is making them aware of people's roles and responsibilities and documenting it.
What is Notification of Rights?
This form is completed annually with families so they know what their rights are and where to report violations. Complete this annually no more than 365 days.
