Kentucky Performance Rating for Educational Progress?
Most classrooms are not equipped to secure test materials under two locks. Therefore, a teacher or staff member should always be present and able to view materials at all time. If a teacher needs to leave the room, they should call Mrs. Trish Green so she can page Ms. Hagan or another staff member to be with the materials in the classroom while teacher is out briefly.
( This will also include any students who enroll during the testing window.)
These exemptions must be approved by the DAC and state officials.

Hand off of all materials to the BAC and then the DAC Materials can only be left unattended in a room under two locks.
Who receives extended time on KPREP?
Answers must be transferred to another test booklet (of matching form). For your own protection, let the BAC transfer all answers. A witness must be involved with all answer transfers. The contaminated booklet must be saved, then securely destroyed once the DAC has given permission. Do not do this yourself or even with another teacher present. Do not destroy any test materials. Let the DAC handle that.
All individuals involved (staff, students, parents, etc.) Content area and test part affected (Math, part 1 B) Specific content questions (if appropriate--do NOT include test questions in report, just the numbers!!!) Record what you learned 4 documents: Testing allegations reporting form Notarized document on school letter head, written and signed by each individual involved (1 form with multiple signatures is NOT acceptable.) Testing Schedule Seating Chart with Room Numbers (BAC must provide additional documents including: signed admin codes and proper test administration forms for proctor, non disclosure agreement forms signed by all involved, student name and ID, etc.)
During one of the scheduled make up days following testing. This is for students who miss several days of testing due to illness, etc.
The recommendation is for a child to make up the missed portion of the test as soon as possible.
