When should your clinical notes be completed?

Before you leave the clinic. If you are unable to complete before leaving the clinic (due to extenuating circumstance), then no later than midnight of the day of the clinic and you should message your preceptor in Oscar notifying them when it is done so they can go in and sign off.

4 weeks before the vacation starts (ideally 6 weeks before the start of the rotation, earlier is better)

Who do you notify when sick? Name 3 steps to follow

-CALL: FMTU clinic (or off service preceptor/education lead). 

-EMAIL: Mark, Priya, FMTU clinic (schulichCFM@hrh.ca) and FMTU preceptors affected OR off service rotation lead and off service preceptors affected. If FMTU clinic, try to review your schedule and advise if patients need to be seen same day or moved to future appts with you.

-No later than 5pm on the date of illness, advise if you can return to work the next day.

Family Medicine Mandatory Assessment of Progress test - non-evaluative, proctored self-assessment test, correlated with success in CFPC licensing exam. Happens 2x/year during AHD.

If your patient requests a service or form not covered by OHIP, what steps should you follow?

-Advise the patient there may be a charge

-Consult your preceptor and the FMTU third party fee list

-Complete the service/form and then take the patient / form to the front desk with a sticky note indicating how much to charge the patient. (Front desk will collect money and give to your preceptor)

When you order a consult in Oscar, how do you indicate who the ordering MD vs. ordering resident is?

At the top: drop down menu with physician's name should be changed to your supervisor of the half day. At the bottom, in the notes section write: "Dr. Resident for Dr. Supervisor"

How often do you get an FM ITER and who reviews it with you?

Every 3 months, and your main preceptor reviews it with you. They use a combination of your field notes and feedback from teacher's meetings to guide the completion of your ITER

If your patient No-Shows to an appointment, what should you do?

- Check how many No Shows they have had by looking at the Appointment History in Oscar

- Decide if you would like to send a No Show letter to the patient

-If you are sending a No Show letter to the patient, message clerical in Oscar: "Send No Show Letter #1/2/3"

How often should you check your Oscar and HRH email inboxes? Bonus: When you are on vacation, how do you notify others you will be away and who is covering?

Check your inboxes every 24 hours (48 hours if post-call). Send an away message in Oscar, and set your away alert in your outlook

Name 4 steps to follow when requesting vacation

1. Consult the FMTU vacation tracker

2. Send in the FMTU vacation request form to Mark and Priya

3. Block your Oscar schedule with a dummy appt "DO NOT BOOK"

4. You will hear back within 7 days. If approved, please send a message to SchulichCFM@hrh.ca advising about your approved vacation and where to move any booked patients. If declined, please remove the dummy appointments and submit a new vacation request.

When can you come/return to work after an acute febrile or respiratory illness?

The criteria for returning to work after any respiratory illness will be as follows:

  • Fever free for 24 hours (without medication)
  • No GI symptoms (vomiting/diarrhea) for 48 hours.

On return to work staff will be required to wear a surgical mask for up to day 10 from symptom onset and to social distance (at least 6 feet) from other staff members while eating in common areas. Staff will have the option to wear an N95 respirator, if that is their preference.

Where do you find your RPP (Resident Practice Profile) and what types of encounters do you enter into this tool?

The RPP is found on the DFCM website, or at: q.utoronto.ca

The RPP is used to determine the breadth of clinical experience in residency training, and for identification of practice deficiencies. 

It is good practice to update your RPP at the end of every family medicine half day, as RPP reports are monitored by the DFCM and you will be emailed if you are falling below the expected level of patient encounters. 

The Postgrad PD will review your RPP with you at your six month progress review meetings. Enter in all FM and after hours clinic encounters. Encounters on Teaching Practice should be entered into RPP as well. Up to three diagnoses can be entered per patient visit
