What is when the LEA is unavailable for 30 month meeting
The last day of this month is the month that the family has to decide if they wish to proceed with, or "opt out" of notification to the school department.
28th Month
This form is completed with the family and sent over to the LEA to notify the school department of a potentially eligible student

The form that is completed by the Service Coordinator to document the transition steps, this form is shared with the school department and the family
Transition Steps and Planning- T-2
The document completed by Early Intervention with input from parents and anyone providing services to the child to reflect the child's PLOD is sent to the school department prior to the referral meeting
