What was the main cause of spread for the Bubonic Plague? How did it spread so easily?

The main cause of the Black Death was fleas that bit both rats and humans. Due to poor sanitation in Medieval Times, the Plague spread rapidly, giving little to no time for a city to prepare for a strike of the illness. Several homemade remedies were attempted to try and rid  people of the disease, but it was useless. Without the proper knowledge of disease transmission, people were doomed and their fate as tragically sealed.
